Frequently Asked Questions
What insurance do you accept?
We’re in-network with BCBSIL under these plans: PPO, BCS, BCE & BCO. All other insurers are out-of-network (private-pay); we provide superbills for possible reimbursement.
When is payment due?
Payment is collected at the time of your appointment. Insurance cards must be on file beforehand. We accept credit/debit, HSA/FSA, and cash.
Do you offer a sliding-scale fee?
Yes—if financial hardship applies, you may qualify for reduced rates. Documentation (proof of income) is required; contact our billing team to discuss.
What if I need urgent help?
We do not provide crisis services. Call 911 or dial 988 for immediate assistance.
Fees & Policies
Billing & Superbills
BCBSIL in-network plans are billed directly. For private-pay or out-of-network, we’ll send you a digital superbill after your visit.
Sliding Scale
Reduced fees based on income—documentation required. Email billing@gravitypsychiatrywellness.com to apply.
Cancellation Policy
Please provide 24-hour notice to cancel or reschedule. Late cancellations or no-shows may incur a fee up to the full session cost.
