How do I ask about becoming a patient?
Call (309) 245-7115. Our team will review current availability, explain the process, and tell you what information is needed. Calling does not guarantee an appointment or transfer prescribing responsibility.

After the office confirms your appointment, you will receive an email invitation to activate your CharmHealth Patient Portal and complete the required forms and questionnaires. Check spam or junk if the invitation does not arrive.

Use the secure CharmHealth Patient Portal for appointments, refill requests, forms, billing information, shared records, and care-related messages. Use Spruce only when the office has specifically directed you to that channel.
Patient portalWhat should I know about insurance and payment?
Gravity works with select commercial BCBSIL plans and offers self-pay services. Coverage and patient responsibility vary by plan and must be verified. A valid payment card on file is required before appointment confirmation unless the practice approves another arrangement.
How do medication refill requests work?
Submit routine refill requests through the secure patient portal before medication is exhausted. Refills are not guaranteed and may require an appointment, monitoring, or other clinical review. Do not use refill messages for urgent or emergency needs.
How do providers send a referral?
Fax the referral and relevant supporting records to (309) 204-8259.

Do not send detailed health information, medical records, identification documents, Social Security numbers, or payment-card information through ordinary email, text, or public website areas. Use the secure patient portal or call the office for the appropriate route.
